Summary:
Under the guidance and supervision of the Program Director, the Lead Case Manager is responsible to carry an assigned caseload, as well as support the day-to-day operations of the case management team, including the ongoing monitoring of timely and accurate submission for all required documentation within all required County, State and SWK database systems.
The Lead Case Manager is required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Knowledge and abilities:
Must be proficient with computers to use Microsoft Suite, SWK's web-based programs, county, and state web-based programs.
Possess great attention to detail to complete assignments with a high degree of accuracy.
Possess excellent written and reading comprehension to document electronic progress notes (efforts) and all forms.
Organize work, prioritize, and can meet deadlines.
Possess excellent communication with external parties and professionals at all levels of the organization.
Able to work independently with minimal guidance and as part of a team.
Duties &
Responsibilities:
Carry a caseload of ten (10) Family Keys cases.
Conduct initial phone contact with client and family/guardian as necessary.
Ensure the timely completion of all Intake paperwork, Assessments, and Individual Service Plans in accordance with Southwest Key Programs (SWK) policy.
Maintain physical (if applicable) and web-based client files.
Establish and maintain therapeutic relationships with clients and families.
Provide crisis intervention for clients and their families.
Provide weekly home visits with clients and families completing thorough documentation in SWK database system.
Ensure clients participate or are offered all components or services offered at the program.
Serve as main point of contact for youth referrals, youth transfers and request for information in coordination with Program Director.
Other Functions:
Attend and participate in all required professional trainings and workshops.
Maintain open and consistent communication with Program Director or designee, and other team members.
Transport clients to program activities, court appointments, school hearings or other treatment meetings as needed.
Able to react to change productively and handle other essential tasks as assigned.
Work evening, weekend hours and be on call.
Qualifications and Requirements:
A combination of one of the following:
Bachelor's degree in a social service or related field with two (2) years' experience working with at risk youth and their families; may include part time, volunteer or internship experience.
Bilingual in Spanish and English preferred.
Clear background checks from all appropriate entities.
Must be able to obtain First Aid Certification (CPR).
Must possess a valid state driver's license, registration and maintain state mandated car insurance.
Physical Demands:
Must be able to verify client's whereabouts by driving, sitting, standing and walking for extended periods of time.
Daily use of various office equipment.
Bending, stooping, climbing up stairs and lifting up to 15 lbs.

Work Environment:
This is a program which requires daily contact at various client locations.
Traveling to various locations approximately 75% of the time.
Must be willing to use personal vehicle for work related purposes as needed.
Work shifts are subject to change.
Noise levels vary from mild to moderate.
This position may require extended periods of time in an automobile.
Exposed to prevalent weather conditions.

The check will bounce and the victim is left responsible.
